Check List
- Student’s Birth Certificate
- Student’s Up-to-Date Immunization Records
- Parent / Guardian Picture Identification Card
- Legal Guardianship Papers (if applicable)
- Two Different Proofs of Orleans Parish Residency (current utility bill, rental lease, mortgage, property tax notice, government issued letter etc)
- Notarized Affidavit of Residency (if both parent and student are residing in the home of another individual)
- Student’s IEP and/or 504 Plan (if applicable)
- Student’s Recent / Final Report Card from Previous School
- Student’s Withdrawal Form / Drop Slip from Previous School (if applicable)
- Student’s Final Standardized Test Scores (if applicable)
- Student’s High School Transcript (if applicable)
- Student’s Social Security Card (optional – if a parent / guardian provides the optional document, it is used solely for expediting funding sources)
